An umbrella review of systematic reviews and meta-analyses looked at 28 different treatments and found that several were associated with improved sperm quality. Some of them may also support your body's own testosterone. Here's the big picture, without the jargon.
First, you're not alone
Fertility struggles are far more common than most people realize. Roughly one in six couples runs into difficulty (this is an older estimate, so treat it as a rough guide rather than a precise figure), and a male factor is one of the most common contributors, involved in a large share of those cases.
The encouraging part: male fertility is often something you can act on, not just something to accept.
What the research looked at
Researchers pooled together systematic reviews and meta-analyses and compared 28 different treatments for male fertility. They measured the things that matter most for sperm: how many there are (count), what shape they're in (morphology), and how well they move (motility).
That gives us a rare, big-picture view of what tends to help, and what doesn't. It is worth saying upfront that the authors rated most of the evidence as moderate quality, so these are useful signals rather than the final word.
The good news: several things appear to help
A few broad categories stood out as associated with better sperm quality:
- Antioxidant-based options. These target the kind of cellular "wear and tear" (oxidative stress) that can damage sperm, and were linked with better count, shape, and movement.
- Hormone-balancing approaches. These gently support your body's own hormone system, which can be associated with improved sperm quality.
- Fixing a varicocele. This is a common, very treatable vein problem in the scrotum that can quietly drag down fertility. Correcting it was linked with improvement.
In short: many men saw measurable improvements. Fertility is often something you can act on, not a dead end.
Antioxidants deserve a closer, honest look, because they are where the best evidence sits. In the largest review to date (a Cochrane review of 90 trials in more than 10,000 men), clinical pregnancy rates rose from an assumed 15% without treatment to somewhere between 20% and 30% with antioxidants, and live-birth rates from about 16% to between 17% and 27%. Encouraging, but read it with care: the certainty of that evidence was rated low, many of the studies were small, and when only the highest-quality trials were counted the live-birth benefit faded. Antioxidants are inexpensive, easy to find and generally well tolerated (the main downside was mild stomach upset), which makes them reasonable to consider, but as a supporting measure inside a proper plan, not a guaranteed fix. Reading that nuance correctly, rather than selling you a supplement, is exactly the kind of judgment a specialist adds.

Here's a point that surprises people. The smartest fertility approaches don't just chase sperm numbers, several of them work by optimizing your body's own hormone production, which can be associated with better sperm quality at the same time.
That's an important distinction. The aim is to help your body make more of its own testosterone, not simply to replace it. Supporting your natural hormone balance is what helps keep fertility and testosterone moving in the same direction.
Where medication fits, if you are not on testosterone
Lifestyle and the targeted approaches above do a great deal. When the hormonal signals themselves are off, there are prescription options, and the first thing worth knowing is that it is rarely one medicine. Fertility usually has several things slightly off at once, which is exactly why a single pill so often disappoints.
If you are not on testosterone therapy, the protocol rests on three pillars: clomiphene, which keeps your brain sending the LH and FSH signals; and hCG and hMG, which speak directly to the testes to drive testosterone and sperm production. Depending on what your results show, add-ons may include anastrozole, to keep estrogen in the right range, and Proviron. Both have some supportive evidence in fertility, and neither is routine; they are used where your numbers justify them, not by default. In Spain, clomiphene is the pharmacy-grade option available; enclomiphene, discussed a great deal online, is not.
The pillars are not the secret. What is done with them is. The combination, the doses and the order are built from your blood work, your semen analysis, your medical and personal history, and what you are actually trying to achieve, which is also why you will not find a finished protocol written out here for copying into a pharmacy order. Precision medicine is precise because it is yours, not because it is complicated.
If you are already on testosterone therapy, the picture changes completely, and clomiphene is not the answer, because outside testosterone has already switched off the very signal it works through. All of these are prescription medicines that require proper assessment, dosing and monitoring by a specialist. None are something to source or self-manage, and the right choice always starts with measuring where your hormones actually stand.
And what doesn't really help
Not everything lived up to the hype. Plain vitamins on their own didn't move the needle. In the review, vitamin supplementation showed no significant benefit for sperm count, motility, or morphology. A standard multivitamin off the shelf isn't a fertility fix, the approaches associated with improvement were more targeted than that.
It's a useful reminder that "natural" and "effective" aren't the same thing, and that guessing with supplements can waste months.

Designing a combined, intelligently sequenced protocol, several evidence-based levers pulled in concert and adjusted as your bloodwork and semen analysis respond, is a real clinical skill. It is where a specialist's experience and physiological understanding earn their place, and it is not what a one-variable-at-a-time approach tends to deliver. Combining agents this way, chosen so their physiological effects complement rather than collide, and sequenced so each one is given the conditions to work, takes training in both internal medicine and endocrinology. It is a more elaborate and more elegant way to practice than the single-lever protocols in common use, and it is the reason men travel for this rather than repeat it locally.
Done well, that means supporting your body's own hormone production, reducing oxidative stress, correcting what is correctable, and tuning the lifestyle factors that quietly move the needle, all coordinated, all monitored, and all built around your specific cause rather than a generic checklist. It will not suit every case, and no protocol can promise a pregnancy. But for the right man, it is a faster and more thoughtful path than trying one thing, waiting, and starting over.
